Job Summary
Northern Care Alliance NHS Foundation Trust is looking for an experienced Social Work Practitioner to join Salford Adult Social Care.
The role focuses on supporting people to live well and safely at home through strength-based conversations that promote wellbeing. Practitioners work closely with local organisations to connect people to community resources and support them in achieving their personal outcomes.

Main Duties of the Job
Key responsibilities include:

Completing strengths-based assessments and promoting wellbeing using resources from social care, health, and other statutory or voluntary agencies.

Undertaking initial assessments and managing complex and legal issues, including crisis planning.

Providing information, advice, signposting, and transferring cases to other social work teams or services as required.

Identifying the need for further assessment under sections 9 and 10 of the Care Act and transferring cases to the relevant social work teams.

Completing support planning and reviews.

Undertaking Safeguarding functions, including initial information gathering, risk assessment, safety planning, and determining whether Safeguarding Adult Enquiries under section 42 of the Care Act are required.

Managing competing demands effectively, prioritising workload, and remaining calm under pressure.

Essential:

Recognised Social Work qualification (Degree, CQSW, CSS, Dip SW, or equivalent)

Current registration with Social Work England

Full, current driving licence with a vehicle and valid MOT certificate

Adequate vehicle insurance cover

Ability to perform social work functions in line with legislation, policies, and best practices (Care Act, Mental Capacity Act, Mental Health Act, Equality Act)

Conduct strength-based assessments, identify urgency, and implement actions while balancing rights
-
Triage safeguarding referrals effectively, considering risk, safety planning, and decision-making (Care Act 2014, s.42)

Prioritise and monitor own workload, completing timely assessments and support planning
-
Develop effective relationships with service users, carers, families, and colleagues
-
Work effectively within a multi-disciplinary team
-
Maintain accurate, complete, and up-to-date records using appropriate IT systems

Keep knowledge of legal, policy, procedural frameworks, and social work methods up to date
