Job Summary
Lincolnshire County Council is offering an exciting opportunity for a Social Worker Level 1 within the Adult Care Team at Pilgrim Hospital, Boston. This role is suitable for newly qualified Social Workers completing their ASYE or Level 1 practitioners seeking experience in a hospital-based adult care setting.
The role focuses on supporting adults to live independently wherever possible and ensuring that individuals who require care receive the right support, in the right place, at the right time. Service users and their families are placed at the centre of all decision-making processes.
The team works closely with NHS and third-sector partners to ensure that individuals are fully involved in planning their own discharge and ongoing care. Creativity and innovation are encouraged when developing care and support packages that empower service users.
Lincolnshire County Council is also leading the introduction of a fully integrated Discharge to Assess Hub, designed to improve access to services, reduce delays, and deliver better outcomes.

Working Pattern

Key Responsibilities
-
Deliver statutory social work services for adults with care and support needs
-
Undertake assessments in line with the Care Act 2014, using a strengths-based approach

Develop, implement, and review care and support plans
-
Manage a caseload with appropriate supervision from the Lead Practitioner
-
Work collaboratively with health, housing, and third-sector partners
-
Contribute to safeguarding enquiries in line with local safeguarding policies
-
Identify non-social care needs and provide information and advice
-
Construct cost-effective care packages that promote independence and quality of life

Maintain accurate case records and produce professional reports

Provide advice and support to other Adult Care practitioners

Give evidence in court when required

Work autonomously while contributing to a multi-agency approach
