Job Title: Care Assistant
Job Types: Full-time
Salary: £13.50-£15.50 per hour
Location: 1 Falcon Way, Shire Park, Welwyn Garden City AL7 1TW
Your role will include:
Supporting individuals as they transition from hospital to home, helping them regain independence.
Visiting service users in their own homes to provide care as outlined in their personalized care plans.
Assisting with personal care, administering medication, monitoring vital signs, exercise, and domestic tasks.
Liaising with medical professionals such as district nurses and pharmacies.
Qualifications:
Valid driving licence and own vehicle
Experience in creating and implementing care plans
Good communication skills in English
Proficiency in IT for documentation and reporting
Ability to provide compassionate home care
Familiarity with care home environments and protocols
